For question 40 if you populate ‘true’ you are required to complete the rest of the question. However, if you populate ‘False’ the rest of the fields will not be populated in the XML.
For question 41 if you populate ‘NONE’ you cannot populate anything in the ‘other’ column. If you populate the risk measure as ‘Other’ you must report it in the other column.
